Testimonial Release Form

In consideration of my participation in the services provided by LunaCco Design, located at 41 W Hwy 14, 221 Spearfish, South Dakota 57783 (“Company”), I, hereby grant permission to the Company and its authorized representatives (hereinafter collectively referred to as “Company”) to use my name, image, likeness, voice, testimonial, and biographical information (collectively, “Materials”) in all forms and media now known or hereafter devised, including but not limited to photographs, videos, audio recordings, websites, social media, brochures, advertisements, and other marketing and promotional materials.

I understand and agree that:

1. Purpose of Use:
   The Company may use the Materials for any lawful purpose, including but not limited to promoting and advertising the Company’s services, products, and events, as well as showcasing its portfolio of work.

2. Scope of Use:
   The Company may use the Materials in any manner it deems appropriate, including but not limited to editing, altering, cropping, combining with other materials, and creating derivative works. This includes use in digital formats such as websites, social media platforms, and email marketing campaigns.

3. No Compensation:
   I will not receive any compensation or royalties for the use of the Materials.

4. Duration of Use:
   This release is perpetual and irrevocable, meaning the Company may use the Materials indefinitely.

5. Right to Use:
   I represent and warrant that I am at least 18 years of age and have the full right, power, and authority to grant the rights granted herein. I further represent and warrant that the use of the Materials as authorized herein will not violate any rights of any third party.

6. Release of Liability:
   I hereby release, discharge, and agree to hold harmless the Company, its officers, directors, employees, agents, successors, and assigns from and against any and all claims, demands, liabilities, losses, expenses, damages, costs, and causes of action, whether in law or equity, known or unknown, suspected or unsuspected, arising out of or in any way connected with the use of the Materials as authorized herein.

7. Ownership of Materials:
   The Company shall own all rights, title, and interest in and to the Materials, including any edits, alterations, or derivative works created by the Company.

8. Governing Law:
   This release shall be governed by and construed in accordance with the laws of the State of South Dakota.

I have read and fully understand the terms of this release and voluntarily consent to the use of my Materials as described above.
